My EPF transfer has been rejected twice citing the below reasons:

wages are more than 15000 still eps share remitted submit service before 1.09.2014 if any or submit proper documents for merger request from eps to epf

date of joining after 01.09.2014 with higher salary more than 15000 plese clarify why pension contribution deducted and member have EPS membership
This issue was prevailing even when EPFO approved my previous transfer requests, but now they are not approving the transfer. I raised CPGRAMS and RTI complaints citing administrative inconsistencies but to no avail.
Can I connect with someone to understand the procedure and get this done? Looking for your support on this matter
I understand revised Form 3A maybe required here. Any template or format for the letter and form 3A? My ex-employer does not have knowledge regarding this so need help for all relevant details
