Is there a format for Probation review form in a IT startup? What are the common parameters included in the list of checking?

Probation is a period of trial and its purpose is to find out the suitability of an employee to hold the post substantively or permanently and the suitability has to be judged at the time of confirmation

Performance Factors
Attendance
Punctuality
Performance (Includes Job Knowledge, output, decision making)
Behaviour (Includes obedience, relationship, attitude etc.)
Integrity (Includes dependability, reliability, honesty etc)
