We had terminated an employee in the Year 2017. As we have an open position in that location, he has reapplied. Can we hire him back. Is there any policy for rehiring terminated employees?

Generally company can rehire any ex employees after 6-12 months. In your case it is more than 3 years so in my opinion you may proceed.
Now talking about termination due to non performance then while rehiring him department head will have to review him for the position and HR needs to reassure about his performance before taking him on board.

For Rehiring of an ex-employee, there is no standard process or policy.
Normally, companies make sure they do not rehire for atleast 6 months to 1 year.
Post that also, when you are rehiring an ex-employee irrespective of Performer or Non-Performer, you should follow your due process of Recruitment and evaluate whether he fits for the role or no. Yes, he is an ex-employee; but evaluation process during interview should be similar to any other Candidate.
