It is a recommended practice to document the internships provided by an organization.
Whether the internship is offered with / without pay, accordingly, you can issue the offer letter stating the terms to which the intern will be subjected to.
Interns as long as they are in the organization, both the intern as well as the employer is accountable for their conduct, respectively.
Keeping the positive as well as negative outcomes that may / may not occur, documenting internships will be a preventive and structured approach.
