To make sure it is a legitimate claim you can collect the below documents along with Form D.
- Medical Certificate from a registered practitioner confirming the maternity.
- Hospital Discharge Summary.
- Newborn baby’s Birth Certificate.
- Form D to be filled by the (Format Attached)
