What are documents, the company should ask from its employee for applying maternity leave?

To make sure it is a legitimate claim you can collect the below documents along with Form D.

  1. Medical Certificate from a registered practitioner confirming the maternity.
  2. Hospital Discharge Summary.
  3. Newborn baby’s Birth Certificate.
  4. Form D to be filled by the (Format Attached)
