Is employer required to issue a Form 16 to all employees as a rule now?

Yes, definitely for all whom the employer deducted TDS
Not for all. Only for those for whom TDS is deducted. Other than that, if anyone asks we can issue Form 12B

Form 16 is technically a certificate for tax deducted at source. However, banks etc are insisting on submission of Form 16 to give even small ticket items like mobile loans etc. Therefore from a practical point of view, employers are filing Form 24 Q for all employees (even those who do not have TDS) and downloading Part A and Part B of Form 16 from the TRACES website. This digitally signed form is then given to all employees.
